Long Term Care

APANO Supporting Long Term Care

A community-based service offers a welcoming and supportive environment designed to enhance the quality of life for individuals in need of ongoing care and connection. The program features a congregate dining service, which provides nutritious and balanced meals in a shared setting that encourages social interaction and helps reduce isolation among participants. Mealtime becomes not only an opportunity for nourishment but also a chance to build friendships, share experiences, and strengthen a sense of belonging within the community.

In addition to dining, the program includes friendly visiting and security checks conducted by trained Long-Term Care (LTC) Workers. These visits offer emotional support, companionship, and reassurance, helping participants feel safe, valued, and cared for. The regular security checks also ensure that individuals are healthy and comfortable, and that any emerging needs or concerns are addressed promptly. This proactive approach fosters a secure environment where clients can thrive with confidence and dignity.

Personalized Services

All services are personalized to meet each participant’s unique health, wellness, and personal care needs. Whether it involves assistance with daily activities, emotional support, or connecting clients to additional community resources, the focus remains on promoting independent living and maintaining each person’s autonomy for as long as possible.

Our mission is to deliver holistic, person-centered care that nurtures the body, mind, and spirit. By integrating social engagement, health support, and safety measures, this program aims to create a balanced and compassionate model of community living. Through collaboration between LTC Workers, participants, and their families, we strive to build a caring network that empowers individuals to remain active, independent, and connected in a safe and respectful environment. Ultimately, our goal is to help each person lead a fulfilling life while receiving the care and support they need.
